As soon as you get the V5 for your GP back from the DVLA take it, your Certificate of Entitlement, the current Tax Disc and this form to your local DVLA office.

You'll probably have to queue for a while, if your local office is anything like mine!

The DVLA bod behind the counter will stamp lots of things and press lots of buttons before handing you the back page of your V5 and a replacement tax disc showing your new number. If you want to be 100% legal make sure you have plates made up for your new number before you go and fit them in the DVLA car park before you drive off, but that's your call. Depending on how 'by the book' your local office is, it might be a good idea to get your insurance company to reissue your certificate showing your new plate to start on the day you plan to do the swap, as they may ask to see it but don't always.
