Separated harmonic balancer

Recently I heard a relatively high frequency pulley noise from my 2003 base. shortly after the serpentine belt shredded. After removing the belt and tensioner. I found the tensioner pulley to spin dry. After replacing the tensioner pulley and belt , the noise went away. After driving a couple of miles, there was bang followed by a loud grinding. After removing the serpentine belt a second time, I found the outer ring of the harmonic balancer had separated from the inner hub. Is this common?

Its a common problem on the early S models, I changed mine to a solid pulley
